Consumer Health Data Privacy Policy

This Consumer Health Data Privacy Policy describes how Bird’s Eye Medical collects, uses, and shares “consumer health data” as defined by Washington’s My Health My Data Act (RCW 19.373). It applies to consumer health data that is not protected health information (PHI) under HIPAA. PHI we create or receive while providing care is governed by our Notice of Privacy Practices.

1. Consumer health data we collect

Information you choose to include in our website contact form or emails that relates to your health, such as a request for a vaccination or health-screening clinic
Information that could reveal an interest in health services, such as visits to clinic-related pages of our Site

Directly from you, and automatically from your device when you use the Site (see our Website Privacy Policy).

To respond to your requests and schedule or coordinate services
To operate, secure, and improve the Site
To comply with legal obligations

4. Sharing

We do not sell consumer health data. We share it only with service providers (processors) that help us operate the Site and respond to you, including GoDaddy (website hosting), Brevo (email delivery), and Google (website analytics), and as required by law.

5. Your rights

Washington consumers have the right to:

Confirm whether we collect, share, or sell your consumer health data, and access that data
Get a list of third parties and affiliates with whom we have shared it
Withdraw consent
Request deletion of your consumer health data

If we deny your request, you may appeal by replying to our decision; if your appeal is denied, you may contact the Washington State Attorney General at atg.wa.gov.
